COMPLETING AND SUBMITTING THE ORDER FORMS
Sign the order form authorizing the order and the release of any patient materials. SaraPath is not able to process your request without a signed order form. Also include a contact name and phone number on the order form, in case we need to contact you for additional information about your order.
Also provide the patient’s billing and insurance information, as SaraPath does not see patients and our information may be outdated.
The SaraPath request forms can be completed and submitted to SaraPath online, by clicking the submit button located on the bottom of each form. The request forms can also be faxed to SaraPath’s medical records coordinator at 941.362.8944. The Moffitt, Foundation Medicine and Genomic Health order forms can also be faxed to SaraPath.
BILLING AND PAYMENT
Some insurance plans may require preauthorization, especially for certain molecular tests, therefore patients should verify coverage with their insurance plan in advance.
